CCS strongly encourages membership in HSLDA (Home School Legal Defense Association) though it is not mandatory for enrollment. If you have an HSLDA membership number, please submit this information to CCS for our files.

CCS requires a quarterly progress report to be submitted for each student enrolled either by postal mail or through an online report form. The quarterly report must be submitted no later than 10 days past the due date. After 10 days a late fee of $5.00 per week will be applied until the quarterly report arrives at CCS. Failure to submit the quarterly progress report within 30 days, without an extension from CCS, will result in dismissal of the student from CCS.

Student Immunization Records and a Health Examination Report or a signed waiver for these must be kept on file for each student. This is state law. There is an area on the Immunization Forum and Health Report Form for parents to sign a waiver if they prefer to do so. Please forward these forms to CCS with your enrollment paperwork. If you are enrolling online, please print off a copy of each form and either have them filled in or sign the waiver and forward by postal mail to CCS.
